Generalist Case Manager

Position Title: Generalist Case Manager
Reports To: Case Management Coordinator
Department: Direct Services
Salary: $30,000 – $35,000
Status: Full-time, exempt, eligible for benefits

Qualifications:

• Degree in Social Work, Human Services, Sociology, Psychology or Nursing. Minimum Bachelor's Degree.
• Experience in social/human service delivery.
• Expertise in case management, client advocacy, and public health.
• Ability to work with diverse populations and with critical and emergency situations.
• Sensitive to cultural and economic differences and the concerns of those infected with HIV/AIDS.
• Knowledge of local resources and service pool.
• Understanding of medical and psycho-social issues related to HIV disease and conditions.
• Flexibility in work hours, including occasional evenings and weekends for annual agency events.

Basic Function:

Provide the full range of case management services to assigned case load of HIV positive clients in clinical and/or office settings.

Specific Responsibilities:

1. Act as clinical case manager for individuals and families infected/affected by HIV/AIDS.
2. Assess clients' needs; develop, implement and monitor service care plans.
3. Link clients to appropriate medical, mental health, addiction and support services.
4. Coordinate and monitor services; assist clients with advocating for entitlements.
5. Provide on-site case management in clinics or hospitals as needed.
6. Conduct home visits as necessary.
7. Collaborate with other service providers and agencies.
8. Provide information, education and referral to clients and members of their support system as required.
9. Serve as a resource for the community around AIDS-related issues.
10. Participate in ActionAIDS Staff and supervisory meetings.
11. Complete written documentation of client interactions in a timely manner.
12. May be responsible for creating and maintaining resource lists.
13. Perform other duties as required.

Performance Standards:

The following performance standards are specific to this position. Meeting or exceeding these standards is an expectation of employment, as is compliance with all other organizational and departmental policies, procedures and expectations.
1. All paperwork on contact with clients and referrals made on behalf of clients completed per time frames established in the Direct Service Policy and Procedure Manual.
2. Document the minimum contact with each client assigned to caseload as per time frames outlined in the Direct Services Policy and Procedure Manual.
3. Maintain a minimum of 400 units of service per month.
4. Participate in weekly individual supervisory meetings.
5. Participate in monthly Group Supervision meetings..
6. Attend Direct Service and General Staff Meetings.
7. Maintain awareness and knowledge of resources, benefits and entitlements necessary to refer clients for services and to advocate for client needs.
